Stallholder Application Harcourts Smith Semaphore Street Fair 2021 The following documentation must be submitted with your application, either via the online form below or e-mailed to email@example.com immediately after. Your application will not be considered unless this documentation is provided. 1. ALL STALLHOLDERS - Certificate of Currency (CoC) Public liability insurance must cover outdoor events for a minimum of $10,000,000 (10 million dollars) coverage. This is a requirement by the Port Adelaide Enfield Council and is non-negotiable. If you do not have insurance cover presently, please email to advise and then send a copy of your CoC once it is in place. 2. ALL STALLHOLDERS - COVID-Safe Plan Please submit your COVID-Safe Plan as approved by SA Health. We require a COVD-Safe plan from each stallholder to be submitted with our COVID-Management Plan. 3. FOOD STALLS - Food Business Notification (FBN) All stallholders selling food must provide their Food Business Notification (FBN). Your Agreement I have read the terms and conditions available here.
